Legal Health Administration - Physical verification for foreigners and overseas Chinese

Applicants: Verification applicants are the four kinds of people below, who are applying for living, working or studying in mainland for a year or more, and who have the physical examination records issued by overseas medical institutions(done within last 6 months):

• Foreigners
• Chinese citizens (Hong Kong and Macao residents)
• Chinese citizens from Taiwan Province
• Chinese mainland citizens settling overseas

The regulation doesn’t apply to children under 16.

Address: Shanghai International Travel Healthcare Center. Ground floor of Building 1, 15 Jinbang Road, Changning District, Shanghai.

Official Time: 8:00~11:00a.m.,Monday to Friday. And, based on the number of applicants, we provide extra service on the afternoons from Monday to Thursday, and on Saturday flexibly.

Reservation: Applicants should make reservation in at least two weeks advance. There’re three ways to make reservation: 1.Reservation online, website:www.sithc.com 2.Or you can do it face to face at both lobbies of Bldg.1 and Bldg.3, No.15, Jinbang Rd.

Documents

1.Foreigners：

1. A valid original passport or travel document, and photocopies of the photo-page, expiry-date-page and info.-update-page of your passport or travel document
2. Four recent 2-inch photos (full face without hat)
3. If applying for working, applicants should also bring a photocopy of your company’s business license or registration certificate If applying for studying, applicants should also bring a photocopy of your students ID or admission notice For those coming to China only as accompanying family members, they should provide a photocopy of family members business license Applicants coming to China for visiting, house-purchasing, marriage, medical treatment or other purposes should bring their real estate certificate,or registration form of temporary accommodation issued by police station affiliated to the local community.
4. Original physical examination records issued by overseas medical institutions(done within last 6 months).

2.Chinese Citizens（Hong Kong and Macao residents）：

1. A valid original home return permit or travel document and photocopies of the photo-page, expiry-date-page and info.-update-page of your home return permit or travel document
2. Four recent 2-inch photos (full face without hat)
3. If applying for working, applicants should also bring a photocopy of your company’s business license or registration certificate If applying for studying, applicants should also bring a photocopy of your students ID or admission notice For those coming to China only as accompanying family members, they should provide a photocopy of family members business license or students ID or admission notice or temporary residence permit to apply for physical examinations Applicants coming to China for visiting, house-purchasing, marriage, medical treatment or other purposes should bring their residence booklet, real estate certificate, marriage certificate, or registration forms of temporary accommodation issued by local police station affiliated to the local community .
4. Original physical examination records from overseas medical institution(done within last 6 months).

3.Chinese Citizens from Taiwan Province：

• A valid original Taiwan Compatriot Travel Certificate or travel document and photocopies of the photo-page, expiry-date-page and info.-update-page
• Four recent 2-inch photos (full face without hat)
• If applying for working, applicants should also bring a photocopy of your company’s business license or registration certificate If applying for studying, applicants should also bring a photocopy of your students ID or admission notice For those coming to China only as accompanying family members, they should provide a photocopy of family members business license or students ID or admission notice or temporary residence permit to apply for physical examinations Applicants coming to China for visiting, house-purchasing, marriage, medical treatment or other purposes should bring their residence booklet, real estate certificate, marriage certificate, or registration forms of temporary accommodation for Aliens issued by local police station affiliated to the local community.
• Original physical examination records from overseas medical institution(done within last 6 months).

4. Chinese mainland citizens settling overseas：

• A valid original passport or travel document and photocopies of the photo-page, expiry-date-page and info.-update-page of your passport or travel document
• Four recent 2-inch photos (full face without hat)
• If applying for working, applicants should also bring a photocopy of your company’s business license or registration certificate If applying for studying, applicants should also bring a photocopy of your students ID or admission notice For those coming to China only as accompanying family members, they should provide a photocopy of family members business license or students ID or admission notice or temporary residence permit to apply for physical examinations Applicants coming to China for visiting, house-purchasing, marriage, medical treatment or other purposes should bring their residence booklet, real estate certificate, marriage certificate, or registration forms of temporary accommodation issued by local police station affiliated to the local community .
• Original physical examination records from overseas medical institution(done within last 6 months).

Certificate-collecting: There are two ways to collect your certificate:

• In person. In four working days after the examination, applicants can go to the certificate-collecting window in person with valid IDs and the examination receipts
• By courier. After the examination, applicants can also sign up for courier services to deliver your certificates to the appointed addresses after four working days. To ensure the safety and privacy of your examination certificates, applicants must go to the certificate-collecting window in person to change the way of collecting your certificates.

Reexamination: We will send a notice to those whose results are abnormal. Upon receiving the notice, applicants should go to the appointed department for reexamination within the stated period of time with valid ID and the examination receipt.
